Forms to Download
Please be sure to have the following forms completed and returned to CCO 30 days prior to your camp session. Having these forms completed prior to your arrival will speed up the check-in process.
All campers must have:
- Health Form- Health needs of our campers are diverse, ever-changing, and demanding. Having health forms in hand prior to your camper’s arrival for thorough review by our health care staff is critical.
- Transportation Form- This form will help us confirm that we have arranged appropriate staff coverage for bus and airport transportation as well as having the necessary arrangements for those parents driving to and from camp. Make certain that all correct names and telephone numbers are listed. CALL the office if there are any changes in transportation arrangements! ALL campers, regardless of transportation needs, must complete this form.
- Parent Confidential Form- This form is used to help the counselor learn about your child. Please help us get to know your child by being as thorough as possible with regard to special circumstances in your child's life (recent family changes, i.e., death, divorce, counseling, etc.) This information is confidential. This form must be completed by a parent or guardian. All campers must complete Page 1. Only Sherpa, Trekker, Leadership Training Program AND Traditional Camp Challenger Campers must complete Page 2.
- Rafting Waiver (ONLY if participating in the rafting day trip. This form will be mailed to you.)
