Camp Paperwork

Please be sure to have the following forms completed and returned to CCO by 5/1/2010.  Having these forms completed prior to your arrival will speed up the check-in process.

              

-         Parent Release & Medical Exam Form- Your campers health and safety are our first priority.  Having health forms  prior to your camper’s arrival for thorough review by our health care staff is critical.

  

-         Camper Information Form & Health History- **New for 2010!** You are now able to complete this portion of your paperwork online. This information will help us get to know your child so we can give them the best expericne possilbe. This information is confidential and this form must be completed by a parent or guardian.

 

 -       Behavior Contract- This form is for all campers age 13+ who are participating in our Challenger, Sherpa, Trekker, and Leadership Programs.  It establishes expectations of conduct during our special teen programs. 

 

-         Rafting Waiver (ONLY if participating in the rafting day trip.  This form is not available online and will be mailed to you. This must be signed, then scanned, faxed or mailed to CCO.)
